For those looking for a streamlined and comprehensive care team for their elderly lived one, who needs a great deal of help but desires NOT to live in a nursing home… this is a one stop place . If over 55, Medicare eligible, and meet criteria for help with activities of daily living – this program covers everything from medical visits, to transportation, specialists, home care, 2 meals every weekday, help with laundry, showering, socialization, physical therapy and end of life care to allow people to live as safely as possible in their own home. Goal is to minimize unnecessary hospital visits and keep people healthier & happier while living longer than in a traditional nursing home model. Average nursing home participant lives 13 month and likely to pass in the hospital. In PACE, average participant lives 4 years and per family and participant request, can pass on their own home with comfort care being offered there. Amazing program with great results in quality for over 35 years across the country.
